Expanding access to high quality care.

The Pollin x The Modern Miracle IVF grant is a gift, not a loan, and is applied directly to an eligible Pollin treatment invoice.

It is available to all who are eligible; regardless of sex, gender, sexual orientation, religious beliefs, or relationship status.

Financial support for one eligible treatment.
Funding is paid to Pollin and applied to the approved applicant’s invoice.

ELIGIBLE TREATMENT

The grant may be used toward 1 of the following:

One IVF cycle, including treatment using donor eggs, sperm or embryo
One frozen embryo transfer using a previously frozen or donated embryo
One egg thaw and fertilization cycle using previously frozen eggs, plus embryo transfer

NOT ELIGIBLE

The grant may not be used for the following treatment costs:

✗ Medications
✗ Donor or surrogate agency and legal fees
✗ Shipping costs of frozen eggs, donor eggs, sperm, or embryos to Pollin
✗ Advanced diagnostic or genetic testing
✗ A grant cannot be combined with an OFP-funded treatment cycle

ELIGIBILITY CHECKLIST

Confirm the requirements before you apply.

Selection considers the program criteria, including household income and medical prognosis. Meeting the criteria does not guarantee a grant award. If a grant is awarded, it must be used within 6 months of approval.

Canadian status

The applicant, and co-applicant, if applicable, must be a Canadian citizen or a permanent resident.

Ontario resident and OHIP

The applicant(s) must currently live in Ontario and hold valid OHIP coverage.

Current Pollin patient

The applicant, and co-applicant, if applicable, must have completed a Pollin fertility consultation and fertility assessment (diagnostic testing). 

Household income 

Annual gross household income must be under $175,000 CAD.

APPLICATION PROCESS

5 steps for financial support.

Applications are submitted through The Modern Miracle Foundation. The fund reviews each eligible application, alongside a medical review board, and shares the decision by email.

Become a Pollin patient

Book an OHIP-covered consultation with a Pollin doctor, attend your appointment, discuss your funding eligibility and complete diagnostic testing for both you, and your partner, if applicable.

Review eligibility

Confirm Ontario residency, OHIP, Pollin-patient status, income, and treatment requirements.

Prepare documents

Complete Assets & Liabilities form, Medical Evaluation by your Pollin physician, and applicant release forms found on The Modern Miracle Foundation website.

Apply and pay $50

Submit the online application and required donation fee along with all of your documentation on the Pollin x Modern Miracle portal.

Allow up to 8 weeks

The Modern Miracle Foundation medical advisory board panel will review your application. Application details will be “blinded” and all identifying information redacted. Physicians do not review applications from their own patients.

If awarded, you will be notified by email.

What is the Pollin x Modern Miracle IVF Grant eligibility criteria?

Applicants must meet all of the following requirements:

  • Both the applicant and co-applicant (if applicable) must be a Canadian citizen or permanent resident and a current resident in the province of Ontario
  • Both the applicant and co-applicant (if applicable) must have valid Ontario Health Care Plan cards (OHIP)
  • You must already be a Pollin patient and have completed a consultation and fertility assessment
  • Combined (if applicable) annual gross household income of <175K
Can I use the grant with an Ontario Fertility Program-funded cycle?

No. The grant cannot be used for an OFP-funded treatment cycle. Having used an OFP cycle in the past does not automatically prevent an application, and receiving a grant does not remove future OFP eligibility, subject to OFP rules.

Am I able to apply for a Pollin x Modern Miracle Fertility Grant if I have used my OFP cycle at Pollin or another clinic?

Yes, you are still eligible to apply for a Modern Miracle grant. However it is important to note that priority consideration may be given to individuals and families who have not yet received, or are ineligible for an OFP funded cycle.

Is the grant a loan?

No. It is a gift and does not need to be repaid. Funds are paid to Pollin and applied to the approved applicant’s eligible treatment invoice rather than paid directly to the recipient.

How much funding is available?

The combined grant provides up to $8,000 from Modern Miracle Foundation and up to $2,000 from Pollin Fertility, for a maximum award of $10,000.

Where do I apply for the Pollin X Modern Miracle IVF grant?

What do I need to provide in order to be able to apply?

All applicants are required to complete and upload 3 documents along with an online application and submit a $50 application fee which goes directly to the fund. Instructions along with the documents and online application link are all found on the Pollin x Modern Miracle IVF Grant Fund page.

What fertility treatments are eligible for the grant?

The grant can be used to:

  • *Offset the cost of 1 IVF treatment cycle up to $10,000
    - Includes cycles using donor egg, sperm or embryo
  • *Cover the cost of 1 x Frozen Embryo Transfer of previously frozen embryo
  • *Cover the cost of 1 x Egg Thaw/Fertilization cycle of previously frozen eggs + transfer (FET or Fresh)

*Applicant is only eligible for 1 grant for 1 of the above treatment options
*All grants exclude the cost of medications.

What expenses are not covered by the grant?

Expenses not covered by the grant include:

  • Medications
  • Donor, surrogate agency and legal fees
  • Transfer fees of previously frozen eggs, donor egg, sperm or embryos to Pollin
  • Embryo storage fees
  • Advanced diagnostic/genetic testing fees
Do I have to use the grant by a certain date?

Yes, grants must be applied to your invoice and used within 6 months of your application approval date.

What is the application review process?

Each application is reviewed by members of The Modern Miracle team, including Medical Advisors. They verify the information provided in the application form as well as the Medical Evaluation signed by your physician.

Personal information in the Medical Evaluation form is redacted to eliminate bias and doctors will never review and opine on their own patients.

How does The Modern Miracle Foundation decide who is awarded a grant?

There are many considerations that go into the applicant selection process. For example, we review financial need as well as the prognosis provided by your doctor. All of these considerations help us with the difficult decision of who is selected for a grant.

How will I be notified if my application was successful?

While the foundation aims to review applications as quickly as possible, it may take up to 8 weeks to complete the review.

Once the application review is completed, you will be contacted via email as to the outcome of your application.

If my application is not successful, can I re-apply?

You can re-apply but we suggest only re-applying if there has been a material change in your application. For example, if your application is rejected due to medical prognosis, we recommend applying if there has been a material change to your treatment plan (eg. from using own egg/sperm to using donor).

If I was successfully awarded a grant, can I re-apply again in the future?

These grants are one-time grants. As such, you cannot re-apply after successfully receiving a grant from the Pollin x Modern Miracle IVF Grant fund.
