1. New Patient Tasks / Pollin App Set Up

  1. Download the Pollin App 
  2. Complete the intake form (Go to the My Journey tab and you'll see a red banner at the top)
  3. Input your home address
  4. Input your family doctor's name, address and fax number
  5. Input your OHIP number
  6. Input your pharmacy details

2. Invite and Link Your Partner

Open the Pollin App > Account > Invite Partners. You can also see this in the My Journey tab as a white banner at the top.

After sending the invitation, your partner will receive a link to set up their account (please ensure they also complete their intake form and add their OHIP number as well). Once your partner has done this, please let us know through the app.

3. How/ When to Contact Us:

  • When you need to reach us, please do so through the chat function (the bubble in the top right-hand corner of the app) or give us a call at 416-342-3552. 
    • This is the best way to get in touch with us as your message will be triaged to the most appropriate staff member based on where you are in your treatment plan.
  • If you need to forward any documents (ex. Results or previous records), you can send an email to documents@pollinfertility.com

4. Post-Consultation

  • Your instructions will be shared via phone call, text and/or our Pollin app Library tab
  • If you need to sign any consents, you can find this in the My Journey tab under Milestones> Upcoming Milestones

5. Results/ Follow-up Appointment

Your results will be discussed at your future follow-up appointment with your physician. Once they are interpreted by your doctor, your Care Navigator (admin) will release these results to your Pollin app (Go to the My Journey tab and open Results). 

If you are completing cycle monitoring, your results will be released the same day by 3:00 pm after it is reviewed by the physician. Please go to the Results tab and click one of the hormone blood work completed that day. From there, you can see the hormone value and the message underneath (the nurse's release note) with your next steps.
