Contacting Us:
When you need to reach us, please do so through the chat function (the bubble in the top right-hand corner of the app) or give us a call at 416-342-3552.
- This is the best way to get in touch with us as your message will be triaged to the most appropriate staff member based on where you are in your treatment plan.
Pollin Consent Forms:
- Please review and sign the Our Care Model Consent form in the Pollin app.
Ordered Testing:
Your doctor has ordered some preliminary testing for you including blood work and a pelvic/transvaginal ultrasound that needs to be completed (on the same day) between days 2-5 of your cycle. For reference, Day 1 will be the first bright red, full flow bleed. Please wait until your menses arrives to book these appointments.
- There are 2 tests not covered by OHIP: Anti-Mullerian Hormone ($125), and Vitamin D ($65)
- You will need a full bladder for your ultrasound
- You do not need to fast for your blood work
After your ultrasound, please go to the Patient Concierge desk and speak to our TNI receptionist to book your sonohysterogram.
Sonohysterogram: evaluates the uterine cavity and fallopian tubes
- Scheduled at 1 pm on from Mondays-Thursdays between days 5-10 of your cycle with one of our physicians
- You will need to take antibiotics as a precautionary measure
- Please let us know what pharmacy to send the prescription to
- Please refer to our Sonohysterogram Patient Handout in the Pollin App > Library for more information on this test
To Book these Appointments:
- Open your Pollin app to the My Journey tab (on the bottom left). Then, under Milestones, go to Upcoming Milestones > press Book Now in each test milestone > select a date and time.
- You can click "Learn More" for more information on how to prep for your appointments
- If any payment is required, you will be prompted to input your credit card information before checking out.
- Once these appointments are booked, you can go to "Required Actions" to confirm you will be attending this appointment.
- Please notify us through the app if you ever need to cancel or reschedule an appointment.
Informative Handouts:
You will be able to find informative handouts in the Pollin app, under the Library tab section.
- There will be a section called "For You" at the top where you may be sent specific handouts relevant to your treatment/plan.
- Feel free to browse through this section as you please
Follow-up Appointment:
Lastly, please let us know via the app or by calling us once you have completed your tests so we can schedule your follow-up appointment to discuss the results. After this follow-up appointment has occurred, we can then release the results to you in the app for your viewing.
